When seeking permanent residence, a green card, or a visa in the United States, you’re typically required to submit a Report of Medical Examination and Vaccination Record– also referred to as Form I-693– to the U.S. Citizenship and Immigration Services (USCIS) department. To complete Form I-693, you must have a medical exam conducted by a designated civil surgeon. Learn more about Form I-693 and how to prepare for an immigration medical exam by reading the detailed instructions below.

6 Steps to Get Yourself Ready for an Immigration Medical Exam in Indianapolis, IN

While the immigration physical is the source of much anxiety for many, there’s no reason to stress! The medical exam is typically simple and ensures that you are eligible to apply for citizenship based on public health standards outlined in section 212(a)(1) of the United States Immigration and Nationality Act.

Follow the steps below to ensure you’re prepared for your upcoming immigration medical exam in Greenwood, IN.

1) Find an Authorized Civil Surgeon.

First, you must find an authorized immigration doctor to conduct your exam. Only physicians that have been designated by USCIS are eligible to perform immigration medical exams. Military doctors are also authorized to conduct these exams, but only at military health facilities in the United States for veterans, members of the military, and designated dependents.

If you’re in need of an authorized immigration doctor in Greenwood, IN or Indianapolis, IN, consider reaching out to Supro Direct! Our lead physician, Dr. Praveen Rajanahalli, is a USCIS authorized doctor with years of experience helping immigrants complete Form I-693.

2) Gather Medical History and Vaccination Reports.

Because the U.S. requires immigrants applying for residence to meet vaccination requirements, it is important to gather available medical records and vaccination documentation for your medical exam. Any additional information you can provide to the civil surgeon will help them to complete your form more accurately and determine whether you meet vaccination and health requirements.

Vaccinations are required to prevent diseases such as mumps, tetanus, diphtheria toxoids, hepatitis B, measles, rubella, polio, pertussis, and any other communicable diseases outlined by U.S. government’s Advisory Committee for Immunization Practices.

Please note that you should NOT try to get vaccinated prior to your appointment. It’s possible that you may not need specific vaccinations due to immunization, or that you should not receive them due to other health conditions. The civil surgeon conducting your physical examination will let you know which vaccinations are needed and can administer said vaccines.

3) Complete Part 1 of Form I-693.

The next step to prepare for an immigration medical exam in Indianapolis, IN  is to complete Part 1 of Form I-693, being sure NOT to sign the form. The form must be signed in the presence of the civil surgeon conducting your exam.

The physician will complete all other parts of the Form at your appointment.

4) Schedule Your Exam.

Once you’ve completed all of the steps above, it’s time to schedule your immigration or green card medical exam. Items to bring with you to your immigration physical include:

  • Government-issued photo identification,
  • Printed Form I-693,
  • Medical records,
  • Vaccination and immunization documentation.

To schedule an immigration physical in Greenwood, IN, please call Supro Direct at 317-300-7424.

5) Attend All Required Follow-Up Appointments.

It is likely that your civil surgeon will require you to attend at least one follow-up appointment. Often, this is necessary to review blood tests, labs, or to administer additional vaccinations. You must follow all instructions and attend required appointments as outlined by your authorized physician.

6) Submit Your Immigration Medical Exam.

Upon completion of your exam, your doctor will provide you with the Form I-693 in a sealed envelope to be submitted to USCIS. It is absolutely critical that you not accept the Form from the civil surgeon unless it is in the sealed envelope. If your Form I-693 is not submitted in a sealed envelope, or if it appears that your sealed envelope has been tampered with, USCIS will return the Form.

Be sure to request a copy of your completed I-693 form from your civil surgeon for your records. For instructions on submitting Form I-693, please visit the USCIS website or contact your local USCIS Field Office.
